File is uploaded but failed to add inside the post

nery	In the post form, I tried to add an image in the post, but it doesn't work. I've checked that the file is uploaded to the server and it can be view by the wordpress admin console inside media. I got the javascript error after upload the file. Below is the screen capture!

nery	Renamed frontend-form.min.js to something else. Now works.
